With your initial counseling appointment, paperwork will be emailed to you to complete PRIOR to the appointment including an Informed Consent, Notice of HIPPA Privacy Practices, an Initial Intake Questionnaire, Consent For Telehealth Consultation, Credit Card Authorization, and Practice Policies.
With your initial holistic coaching session, paperwork including a Coaching Contract, Credit Card Authorization, and Practice Policies will be sent to complete PRIOR to your session.
Cancellation Terms:
- Cancellation received at least 24 hours before the appointment time, a full refund will be issued . If cancelling the appointment yourself please email counselingwithcaitlyn@gmail.com for a refund as our online system does not carry this out automatically.
- Cancellation made within 24 hours of the appointment, no refund and no customer credit will be given.
- No Show for any reason forfeits your appointment.
- Late arrival for appointment will result in a shorter appointment time due to other sessions potentially being after yours. If you are more than 15 minutes late, you forfeit your appointment and no refund will be provided.
